Consular services are available by appointment only.
Only the money order or certified cheque payable to Polish Embassy in Ottawa are accepted.

Please make sure you familiarize yourself with the consular procedures explained on the Consular Services page before scheduling your meeting.

If you would like to meet with the Consul to apply for a passport or for any other consular services, please send your request to polishconsulhalifax@gmail.com and provide the following information:

• Reason for the meeting
• Your full name
• Your email address and phone number

If you are applying for a passport, please provide the additional information below:
• Date and place of birth
• PESEL number
• Number, place of issuance and expiry date of your last polish passport
